Hi Faiv and Welcome.  I have edited your post as we generally prefer people to consult with one of the mods before asking people to participate in online surveys.  I am sure you can appreciate that often people are undergoing treatment and can be unwell and this is a measure we feel the need to undertake on their behalf, to ensure that it is going to be in members best interests.  Thanks.
