Hi Davide,

Yes, in clinical trials people with Gen 1a had a higher response rate than those with 1b.  In the USA of course it`s a different situation because Harvoni is available and also Abbvie`s Viekira Pak, which is why they are now the primary recommendations for all with Gen 1 in the AASLD guidelines that Tig posted in his reply to you.  In India, Europe and elsewhere we are running behind.

So I can see that you don`t want to wait too long before you start treatment then.  What does your doctor recommend?

Hi Jill,

Yes I did FIBROSCAN was F3  and ultrasound showedslightly enlarged liver, but no cancer, spleen enlarged ,other organs normal.

It's a bit confusing as one friend in Amsrerdam is starting Sov./Ribravirin for 24 months, and he is 1A,but I am 1B, Is there possibly a difference in treatment because of the 1a and 1b?

Hi Davide,

I thought you may be interested in the current AASLD recommended treatment guidelines for all genotypes. Best of luck and welcome from me too!

Hi again Davide,

Ok, yes I understand.  Well depending on the condition of your liver you quite likely have time to wait for Ledipasvir to become available.   Do you know what your liver status is, have you had a biopsy or fibroscan test done at any time?

Hi Jill, thank you for verifying this. I checked and Harvoni not yet available in India. I was able to have a friend go to the manufacturer and pick up sovaldi. I'm trying to find a mate that works well with this.

all the best

Hello Davide, welcome!

Yes,  your doctor is correct, as you have Genotype 1 a combination of sovaldi and ribavirin would not be recommended without interferon otherwise it would not be an effective enough treatment.  Unless you were considered to be `interferon ineligible` it`s not an approved treatment option for your genotype..

And the results from the clinical trials indicate that you would most likely stand a higher chance of success with Harvoni (Ledipasvir + Sovaldi) anyway.

Gilead Sciences has agreed licensing deals with various generic drug companies in India for them to manufacture their own versions of Harvoni as well as Sovaldi but I`m not sure what the current situation is regarding availability.   I`ve been looking for some up to date information about that and will post anything I find.

Do be very cautious about where you purchase these drugs from though, it`s better to buy them direct from the manufacturer than from an online website as that`s the only way to guarantee the quality.

Wishing you all the best of luck!

~ Jill
