Need Input on Meaning of Statement in Denial letter

I am working on my appeal for Harvoni and one of the 3 reasons for denial was: "the requested coverage for Harvoni for a member with hepatitis c is not available" So what the heck does that mean exactly? I'm not sure how to respond to that comment. Any suggestions?
