Thanks Malcolm and Wayne, I wasn't sure which test I had done.  Found out more today after work:

                                                             Value                                 Reference range

HCV RNA Quantitative Real Time PCR          16                                   <15  IU/ml

HCV RNA Quantitative Real Time PCR         1.20                                 <  1.18 Log IU/ml 

A <15 Detected means the assay was able to detect HCV RNA but was not able to accurately quantitate the viral load. A <15 Not Detected means the assay did not detect HCV RNA (Target Not Detected).This test is performed using the COBAS® AmpliPrep/COBAS® Taqman® HCV Test v2.0. The lowest viral load this assay can accurately quantify is 15 IU/mL, but the limit of detection is 10-13 IU/mL. Therefore, we can qualitatively report detection even if the viral load is under 15 IU/mL. This is compliant with the package insert guidelines.

This test was done by Quest and while I'm very close, no cigar  smile   I suspect and hope by now I am UND as this was drawn a week ago tomorrow.  Either way no worries, have toooo much to be Thankful for. Really appreciate you sharing your knowledge and experience.  This is a great forum with incredible people.  Just so Gatorman doesn't feel left out  GOOOOOOOOOOO GATORS.  Yes I'm nuts for Gator sports as well. My daughters graduated from U of F and are in the medical field.

Hi Greg,

As you can see, the result needs to be read with the LLOQ and LLOD for each test. So you'll never see a 0 VL. You are just quantifiable but close. Your Lab's Reference Range means the LLOQ is 15. If it doesn't give a LLOD, you assume that's also 15. Best of luck for the Undetected next time. Cheers.

First of all, congrats on the results. Great news! I'm going to refer you to an older forum thread that discusses this in some detail:

My lab uses a Cobas/Taqman test with a lower limit of quantification (LLOQ) of 43 and a lower limit of detection (LLOD) of 7.1 IU/mL for genotype 1, meaning it can accurately measure viral load down to 43 IU/mL. It can detect, but not quantify, viral levels between 7.1 and 43. A result of <43 Detected means the virus is there but can't be measured. It's possible your assay's LLOQ & LLOD are the same @ 15 IU/mL and you would be UND at <15. Others would know better than I.

So I got my latest VL lab results today  smile  and while I think I am understanding this correctly, hoping someone can confirm my interpretation.  My hcv rna quantitative real time PCR is 16 IU/ml.  The reference range shown is <15 IU/ml.  Does this mean that if it were 15 or less it would be considered undetectable or is an actual 0 count needed to be considered UND?  I was so excited to see the results today I didn't ask this specific question of my doctor.  Thanks

Thanks for the response, Greg.  The numbers game can get kind of confusing.  VL and ALT/AST counts have a tendency to jump around.  My VL has fluctuated between 1.2 and 15.6 million in the last 8 yrs.  It is not predictive of the level of liver damage, but used more as a guideline for treatment response.  ALT is more predictive of liver inflammation than is AST, but both can be high w/ little liver damage or low w/ considerable damage.  It's more the total picture that counts.

I've never had a bruising problem and my platelets have dropped into the 40's from time to time.  I wasn't considered for the clinical trials in Seattle last year because I couldn't meet their minimum platelet requirement (60k).  Take care and best of luck..

Congrats Wayne, lookin good.  I know it must feel great to see the VL, almost UND and the ALT, and AST in the normal ranges.  These meds really are the cat's meow.... I'm hoping a miracle in the making for all of us.  Odd how this virus affects people differently.  My ALT and AST were higher than yours at baseline but my viral load was lower?  Not sure what would cause that?  My platelets are also low and a bump to the lower arms leaves bruising/purpura immediately.  Every time I start to get my left arm cleared up (which takes quite a while) I bump it again in the same darn place and start all over again.  no    I pray we get our health back, and this will be the last time any of us will have to deal with this nasty virus.    Be well.

Thought I'd post some lab info through first 6 weeks of tx (S/O/R combo).  Some may find it useful. Any comments and/or number comparisons appreciated, particularly regarding newer combos w/Riba. Clearly levelling off, for most part, with small upturns in Hb/HCT. Thanks!

Just got back with my EOT meeting with the doc. He gave me a hearty handshake and said well done. Still UND at 24wks and he will see me at my 6week post tx follow-up.

Labs look good. Everything within normal except WBC - 3.7L (3.8 - 10.8) and RDW 16.8H (11.0-15.0)

AST - 19 (10-40)
ALT - 21 (9-46)
RBC - 4.89 (4.20-5.80)
Hemo - 15.7 (13.2-17.1)
Hema - 48.3 (38.5-50.0)
MCV - 98.6 (80.0-100.0)
TSH-2.34 (0.40-4.50)
T4 - 9.9 (4.5-12.0)

Looking forward to recovering and getting back top normal.

Doc called today....8 weeks...still undetected.  Blood counts are dropping slowly, but nothing major biggrin

Hey Hillary

Its good advice to empower yourself with as much accurate knowledge about your particular HCV therapy as possible.  Also never ever be shy about asking any question you think of to your doctor.  Having a specialist / Hepatologist also is a very good because they are typically more up to date on the latest HCV developments.

Your numbers are looking good, keep up the good work.

Thanks for all your help.  I appreciate it. I try to read up and learn more about what will happen or what to watch for- I even ask the doc about these numbers and it seems like I just don't remember much of anything lately. hmm I saved a site you posted as a link before about each individual thing they test us for but just don't know what's considered bad. Bad enough to do something about I mean at least.. I really am thinking more about seeing another dr.. But really thanks for all your input. It helps me.

Hi again Hillary,

This is not a medical Forum and we cannot give individual advice. You must find an Hepatologist you trust and be guided by them. When your Hb drops below 10, only your doctor can decide whether to let it drop further, or lower your Riba dose, start Proctrit and advise whether transfusions are required. There are general protocols in place, but some doctors vary in individual treatments.

Some of us make comments when a protocol seems unusual. These are only our opinions and you must decide whether they are worth considering. Try to read up on HepC and be as informed as you can so you can question your doctor. However, there is a lot of nonsense written about HepC on the Internet.

Finally, don't worry about lymphocytes. These go up as the neutrophils drop. See, I can't help myself............

My hemoglobin has dropped quite a bit and seems like it does more as time goes on, what is actually too minch of a drop? The dr said it would happen and it's expected, just wondering how low is too low? And lymphocites are high- not sure what that is either? Well I mean I guess I don't know a lot of it but if something was too far off I would know from the doc or you all right? Thanks for all the help!

I am working on trying to see another dr. I've thought about it a lot and ur absolutely right, I don't wanna put my body thru an extra 6 mo worth of treatment that I don't need. So my boss at work has been thru treatment and she has a different dr she told me about before so I'm gonna get the info from her- LFTs i think those are in this one right?

Good numbers. As Tig said, you can expect a further drop in Hb, RBC and WBC and an increase in MCV.  Those are all Riba effects apart from the WBC.  Victrelis tends to increase the Riba effects. Let us know the next results, and post your LFT's as well.  I hope you're getting a second opinion about treatment duration.  Cheers.

Hi Hillary, those are very good numbers but you'll start to see them decrease now that you've started the Victrelis. Keep them posted here and we'll be able to monitor your progress together. Good luck!

Those are good Kel! I think your WBC's are getting low. They started me on Neupogen when mine reached your level. Be cautious around the sickies! You're susceptible to all of it right now. I'm living a life of avoidance these days, I avoid everyone!!

Hmmm, ok.  Will see what my doc decides on that one. Maybe something else to think about.  Sounds like the treatment can wreck its own havoc on the body system so have to make sure things are in the best working order.

My wbc has been low, just outside the normal range, for years off and on.  They could never figure out why.  Then a couple of years ago was when I found out about the HCV.

My doctor referred me to a hematologist/oncologist before treatment to make sure nothing else was wrong.  After 10 vials of blood, the Hematologist said it was probably caused by the HCV.

Observational Question here . . . it looks like a low WBC is not uncommon, with the Hep C?  Mine is 3.1 and went up in the last month to 3.5 so . . . so?  My doctor is wondering if there is some other underlying condition.  Monday is the biopsy.  Trying not to worry about anything.  confuse
