For HCV patients, ALT is the most important. Interferon has a direct effect on hepatocytes, which may lead to an increase in ALT. It does not mean continuing hepatocyte damage or death. There are 3 published papers on this- they are old but relevant. The conclusion is that fluctuations in ALT during Rx does not affect chances of SVR. It is a concern if the ALT was normal before and in the early part of Rx, with a sudden rise to more than twice normal in the late stage of Rx.

Same things happened to me today... i'am also using triple treatment with incivek i am at 9thweek of treatment right now..

at start of treatment: alt-ast: normal

my 4th week results: alt-ast: 28-19

my 8th week results: alt:74 ast:42

i asked my professor about this sudden increase?

he told me that generally at 3rd month of treatment this is normal and good... ncrease in alt-ast values means, infected partical of liver is clearing. No need to worry anymore.. :)

My enzymes went up and down only a few point but flucuated during tx and i achieved SVR!biggrin

Thanx, that makes me feel a lot better. You can't seem to get a good answer unless you talk to someone else who's been thru the same. That's why i value this forum so much.

Hi Mark, Your fluctuations in enzymes is small, and may even be due to lab. error. Although it would be nice to see the enzmes normalise, it is not a big deal. My enzymes are still slighty elevated, but according to my doc, some people never get normal enzymes, and this has no bearing on SVR. Some patients, particularly if fibrotic, release enzymes from hepatocytes as a direct effect of Interferon. It does not mean continuing cell damage or death.

Starting incivek treatment my VL was 7.9 million, ALT 101 and AST 55. At 8 weeks VL was UND, ALT 129, AST 58. Now at 12 weeks VL still UND, ALT 139, AST 65. Has anyone else experianced or heard of an increase in ALT & AST during treatment? I've always been under the impression that these numbers should go down during treatment. Or is it still too early in TX to be overly concerned with the increase? I'm to see my GI next Friday and I want to mentally prepare myself if there may be some bad news comming.
