I am hoping that the FDA and the Insurance companies realize that 24 weeks of treatment with the Gilead combo is still less expensive than a liver transplant,

Check out this quote from the article that could influence standard treatment protocol.

"The difference between the rates of response among patients with cirrhosis who received 12 weeks of treatment and the rates among patients with cirrhosis who received 24 weeks of treatment was significant "

I know my Hepatologist will be recommending 24 weeks for me, and I am hoping that it will not cost more than the current combo (Sovaldi & Olysio). Who knows in 6 months what the price will be with all the pressure that Gilead is experiencing with Sovaldi.

Whatever price of the new combo will be, the SVR high percentage is worth the fight to go for it.

Good stuff, Matt.  Nice to see this level of detail on one of these studies.  Additional to Bruce's point, glad to see that including riba had no effect on outcomes, either 12 or 24 week.

Matt,

This is a very interesting article. It's looking more and more like we cirrhotics really stand a much better chance of SVR  if we could get 24 weeks of treatment going the route of SOF/LDV. A 99% cure rate w/24 wks of treatment vs 86% cure rate w/12 wks of treatment is quite a bit of difference. Unfortunately for now we may only see people getting 24 wks SOF/LDV in clinicals and not in the real world of insurance.

GT 1a, was tx naïve, cirrhotic(Fibrosure), 64 y.o. Started Harvoni 12/12/14 for 12 wks. EOT weeks 4, 13, and 24 HCV not detected! HVC not detected EOT plus one year.

A smooth sea doesn't make for a skillful sailor.

This is Great article that Gilead and most of the Trials site Doctors have collaborated on that was released at the EASL Conference in London this week-end and also was released as a important article in The New England Journal of Medicine on Sat.

This has both 12 week and 24 weeks results for both cirrhotics and non-cirrhotics.

This report has a Supplementary Appendix and it has some very insight information (For example) Geno 1A cirrhotic patients that completed treated for 24 weeks had 100% SVR-12 Rates
