Thanks for the encouraging words.  We will see the glass half full.  The NP also mentioned some anti-fibrotic concoctions are expected in the next couple of years.  That could be helpful to a lot of people too. Lot of hope.

Hi Ev,

I have to give you something to consider and something that may provide Joe some food for thought. Consider that he's reacting appropriately to treatment first of all, that's excellent all by itself. Second thing to consider is that following SVR, there is documented proof that the inflammation and fibrosis begin to improve. Just because the NP said his liver is now back under the rib cage doesn't necessarily mean it's a negative. It's entirely possible that his liver is reacting very positively to the treatment and the reason for the change in liver size is due to a reduction in his inflammation. That to me is one of those "glass half full", not "glass half empty" references. It may be reason to celebrate, okay? Chin up!!!

Oops I forgot to answer you that his ALT and AST are in the mid normal range now.  They weren't terribly high before but the Dr. said that in late cirrhosis, it sometimes lowers these numbers but not for good reasons.  I can't remember the exact explanation.  He also said the viral load can become lower because you are running out of hepatocytes, also not good. We better just focus on the good news...Non detectable!biggrin

Thank you for replying.  Joe still feels good but maybe a little bit more fatigued.  His Hgb also dropped some.  We saw a nurse practitioner for his 4th week and she was not overly concerned about anything she saw.  She said the TX can cause a bit of suppression of bone marrow.  She said she wouldn't worry about platelets until the 20's and only then if it was accompanied by easy bruising. Joe recently went fishing at night, slipped on a rock and cut his head and also got hit in the forehead with a lure which also did some bleeding.  Both clotted after about 30 minutes so I guess he gave that a good test.hmm It healed quickly.

The N.P. also poked around on him and felt his liver and spleen.  I was asking questions and she was telling me what she was doing.  She made the comment that the liver is shrunk up to the point that it is completely underneath the rib cage which can happen in later cirrhosis.  I think this depressed Joe to hear because he was very quiet for the rest of the day. I wish I hadn't asked.  He isn't one to express it but I think it bothered him. 

I don't want to sound ungrateful for being undetectable at 4 weeks.  We are really just hoping to see this  turn around and avoid a transplant.  There is plenty here to be thankful for.  He isn't a zoned out zombie with nausea and diarrhea for 48 weeks like he was on interferon.  Olysio and Sovaldi are a God send to be sure.

Hi Ev,

Yes, it does say that. Remember, it was a small study, but all 1b's reached SVR 12. How has your husband been feeling? I recall you mentioning he felt early in tx that he was already feeling better. The platelets do bounce. Mine jumped from 60 to over 80k the first month, then dropped back down. I wouldn't be too concerned about his numbers moving around during treatment. The main thing is that it's working. What have his liver enzymes been like (AST/ALT)?

Do I understand correctly that everyone that had geno 1b obtained SVR no matter how much fibrosis they had?  Were the 7% that didn't get SVR genotype 1a's ? I hope I'm not asking a very dumb question but I want to now bad enough to risk it.  :>)  It does say 100% doesn't it?

  My husband is geno 1b but has had cirrhosis for more than 10 years.  He became undetectable by 4 weeks but some of his lab values have worsened.  His platelets were only 44 to start and are now down to 36.  His albumin has also gone down.  After seeing everyone elses improving, it has me slightly concerned but still really happy he became undetectable by 4 weeks.  He was down to 49 by 2 weeks so he certainly seems to be responding but it hasn't helped his lab values as of yet. Still hoping and praying his liver will begin to recover.  I think he was pretty much on the edge of the cliff.
