 If I am correct in my understanding of Ledi is ,That it is already effecting proteins wile being digested and absorbed in the intestine .  And Diet may be a factor that can effect the RAV's .

  From what i have made out so far the ledi inhibits the ns5b protease in 1a but has less anti-viral activity in other genotyps.

The  transporters are effected by ledi at the digestive level and i find it interesting that it causes metabolic stress.  Still looking over it.

Hi all,

Harvoni is a treatment for Genotype 1.

For those who have PM'd me, please read 'Antiviral Activity' on page 17:

'Ledipasvir has substantially lower activity against Genotypes 2a, 2b, 3a..........'.

The Geno 2's and 3's will have to stick with Sovaldi and Riba.

I'm still trying to get my head around the RAV's mentioned. Cheers.

Geno 1b, IL28B CT,  x3 prior relapser,  ex-cirrhotic, 75 yo, did 48 weeks with Victrelis/Peg./Riba.  VL 1.28m at start, UNDET. at 8 ,12 ,16 ,24 ,30  and 48 weeks.  EOT 15 Feb 2013 , UNDET. at EOT + 28 weeks. SVR!  Still Undet. at EOT +5 years

Dx. 2005-liver bx.: stg 2/gr 1. at that time  - GT 1a multiple transfusions in 1981.  Started Sovaldi and Olysio 1/16/14  (No prior treatments) Q80K present.  UND week 4,8 and at EOT.   UND at wk 4EOT, Und at wk 8EOT  SVR 12!!!..SVR 24 :-)

I am seeing quite a bit of information on RAVs contained at pages 17-19.  Hopefully Malcolm will be kind enough to translate for us at some point.  

This information may be particularly relevant to re-treatment issues.

Thanks, Skruffy.  I will be reading that thoroughly but noticed straight away that 24 weeks are recommended for tx experienced cirrhotics.  

I've been waiting to see the duration recommendation from Gilead.  Looks like I know what I will be doing for the next 6 months.  That recommendation should help with the insurance for all gt 1s who are in the same situation--hopefully the full 24 months will be approved for all cirrhotics who need it.
