RC-   I didn't totally follow all of Canuck's reasonings, but I do agree with her and Tig that you have an UNDETECTED and that is fabulous news!  Keep on doing what you're doing and I hope this will all be behind you soon!  You have a lot of people rooting for you and sending you positive vibes and prayers!

That’s the thing with procuring the Indian generics, all you need is a prescription from your doctor. Insurance wouldn’t reimburse anyway and the cost, comparatively mind you, is negligible for the extra weeks. If required, I would certainly ask your doctors impression and approval. If they don’t think it’s wise, follow their sound advice. I’m with Canuck, you’re in mop up stage and this is perhaps completely unnecessary. We’ll work on that premise for now!

I am glad YOU are going to bat for YOU (just as you have always done), keep doing it, and I am glad that your doc's are going to bat for you!

You certainly deserve the insurance and reassurance of another 4 weeks of vosevi. 

I somehow thought your extra 4 weeks (12 + 4 extra) 16 weeks of vosevi WAS already approved? So the first, and second, and now this current "peer to peer" third appeal by your doc is to get you a total of 16 weeks? Can you clarify that? If this is how it is, then I am betting your doc will get approval, with the peer to peer, to add 4 weeks to your 12.

It was unfortunate you had to add riba to your regime, if it could have been avoided, but every bit helps (for insurance, into all you have invested).

The current black and white "paper" recs do say riba is to be added to vosevi in some cases, they do not speak to lengthening vosevi beyond 12 weeks. I do understand your wish for 16 weeks of treatment, even though the 12 plus riba is what they offer, and, for all intents and purposes the 12 weeks of vosevi has already cured you (even before the riba was added).

But I like your plan thus far.

a) wise to add the riba at the 8th week (out of 12), just because.

b) continue with the docs appeal's to add 4 more weeks of vosevi to the 12 (16 weeks) until this avenue is exhausted

c) meanwhile, pre-arrange procuring (on your own) via Gileads assistance program, to make your course 16 weeks. IF THIS IS a viable option that you can make happen (as your back-up plan) - then I would pre-arrange for the 7000.00 option, if possible. That's the price you would have to pay  in order the gain this extra insurance and reassurance. You should not be having to pay out of pocket for this, but, if this is all there is, then ...

d) next best back-up plan would be the good idea indian epclusa (to make it 16 weeks) and add whatever 3/4A your doc dictates/approves of, but this would be time sensitive and approval sensitive also.

e) and, of course, now that you are on riba, you and your docs would also likely have to pre-arrange for you to continue being on riba (for when you succeed in getting the additional 4 weeks) - over the remaining balance of the 16 weeks.

You're on it RC, keep digging and pulling (as you always do) - you will fret regardless of the facts that you are showing every good positive indication that the vosevi has already done the job, very well, for you. 

What a wonderful relief it is to see your first UND RC! C.

First of all, DID YOU SAY, “UNDETECTED”?? We’ve got WOO-HOO’S happening all over this morning!! Big congratulations on this very good news, RC! Finally...

All of the denials and your constant battle to get what’s necessary is damn aggravating. You shouldn’t have to keep dealing with these issues, but I also know you’re not the type to take no for an answer. Just look how far you’ve gotten and you’re not done yet. Keep your head up and eyes on the prize. Just like always my friend!

I’ve got some people looking into your Vosevi supply chain, too. The Indian Pharmaceutical Machine is working steadily on its generic equivalent and getting close, but not close enough to meet your deadline. I have a question though. Ask your doctor if you could use Epclusa, there is a generic available for that. If the NS3/4a is required, could you substitute another, off label? There are others readily available. Now that you’re undetected and on that effectively evil drug, Ribavirin, Epclusa alone might be enough. You’re not cirrhotic now, that’s another consideration. Non cirrhotic GT 3’s don’t necessarily require Ribavirin. We all know your case is unique though. I’m just throwing things out there, maybe something will stick!

Greg Jeffreys has some info on the Epclusa generic. I’m waiting to hear back from Nirav on options as well. Don’t worry Brother, I know we (you) will get this figured out and we’ll all share a collective sigh of relief!

The Vosevi Train continues down the track, each day is one closer to the prize! You’ll get there, I have no doubt...

Good morning all, Its Monday morning 5:00 am. I had to set another alarm for 5am&5pm to take my ribaviren, yep thats right I said Ribavirin. 1200mg a day split into two doses of 600mg each. This will be my third time treating with RIBA, but It will only be for 8 weeks. Dr said because of my past slow response to attain UND it is necessary.(still detected at week 6-7) It looks like 24 weeks of treatment aint gonna happen. Im having to fight my insurance company for the extra 4 weeks as it is! My Dr sent in the RX for the extra 4 weeks of Vosevi and they denied it,  He then appealed that denial and was denied again. So today he has a scheduled peer to peer phone call with insurance companys Doctor. My Dr will plead my case and we will see where it goes from there. The next level of action is a written appeal that goes to arbitration and that will be the final decision. This third level appeal takes some time and I may run out of it? As you all know I cant skip a day of my Vosevi. I have contacted Gileads patient assist program and with there help the 4 weeks of Vosevi will cost me $7000.00. I guess thats better than $29,000.00 for 4 more weeks!! I looked into the prospect of buying the drug from India  but was told by a reputable source that Vosevi is not available yet. It will be 7-8 months before it is manufactured over there in India. I should know in a day or two how the peer to peer turns out.

The trip over to Seattle a week before last was pretty uneventful. All my numbers look good for now. Lfts both low normal 12 and 23 . They ordered a ultrasound of abdomen and liver (done last week). It all looks ok to me however I havent discussed the results with my Dr. The tech who did the U/S said I have nothing to worry about. No liquid in abdomen . Turns out that the pressure im feeling is a result of belly fat! I have been sitting around for the last year and a half and developed what I want to call Thick skin!!!

So last week I had my 8 week blood draw and got the results Saturday. All the numbers look good, the white blood cells and neutrophils are low normal.They did a viral load and it came back undetected.(finally).  I really need the extra 4 weeks of treatment.That would give me 8 weeks of treatment  being in the undetectable Range. Thats the good news bad news of the last two weeks. Sorry I havent posted in a few days, I just dident have all the test results. Back soonRC

Hey bro,

How did the long haul to the wet emerald green city go?

How did the visit/checkup with docs go? Do they do a basic physcial exam everytime? Weigh you. Disrobe you into one of those paper outfits, only to lie down in another nice crinkley sheet of paper, palpate your adb., call their co-horts in for shared lookie-loos, make you stand on one leg, I bet one of the few things that might NOT have changed since you have been going there is your height! But ya never know!

Do you drive over the day before, for doc appoint and stay overnight and then go back home the next day after your appointment?

I got a fill-in doctor my last "follow-up" appointment, nice guy, but same very quick appointment MO style, I gotta be clear, concise, prioritized with my burning questions and requests memorized (impossible for me!) I tend to prattle on whilst he just carries on doing his abbreviated version of a full physical exam which only distracts me further from trying to convey what I want to convey to him! It wasn't 'til hours and hours later, after my visit with him, that i twigged to something he did ... while I was prattling on and he was trying to get his "physical done", he asked me to extend my arms straight out and rotate my hands and wrists upward/skyward and then downward. I was thinking later, now, that's odd, to be doing that kind of neurological assessement on me, no nose touching, hand grip or eyeball/tongue movement components involved, no quizes on my tinnitus/double vision/fugue states, just the wrist rotation thing .... then, Duh!, it hit me ... dummy, it had nothing to do with any kind of neurological assessment, he was simply looking for needle tracks up and down my arms!!! How insulting - but true, this was the first time he had had me as a patient. They get lots of folk through their office where it would be wise, perhaps "standard" to be looking for needle tracks. But, really?!, bro-ther!! How dissappointing, when I had so many other things I wished to be addressed. Oh well, they have their wiley ways of ensuring they ARE doing their due diligence.

Did you get any relief or satisafction with this last visit? Hope so. C.

Yah, I don’t think I’d bet against that either! I have a feeling that Gilead and your doctors will be ahead of this before such a decision has to be made. None of you want anything but the right outcome and I’m confident that’s what you’ll get.

In the meantime, I’m putting a stamp or two on this and mailing it today. You can keep it for your trips to the Emerald madhouse!

Hm, the 24 weeks vs 16 week bet isn't really one I am wanting to wager on, I figure my wager is ... on you going UND (right SOON)! That's a sure-fire better bet for me - saaaaaay, like ... how about a million bucks on that one, OK?

So ... IF you get an 8 week VL, and IF it is UND, and IF they say NO to 24 weeks in their infinite wisdom ... how are you going to feel about that?

UND is UND! - especially now that we are talking new DAA triples like vosevi (not the old stuff!), we are not talking peg or sof/riba/dacla here. Vosevi IS your magic.

Prior failures "set you up", and trials (such as like ALL you have been through) are unnerving. You are raw nerves - stay the course my man, you ARE doing it. I Know that these nerves are (really) nerves of steel that you possess. Jubilent days of relief are coming. 

You are doing a great job, it IS going the right way, this stuff IS unerving, but all your army is back on-line, and vosevi will NOT fail you. winkbiggrin C.

Hellow All, I have blood draw tomorrow (wed) morning, then going over to Seattle on Thursday for clinic. Just the usual blood work this week, no V/L.  Tomorrow will be 7-1/4 weeks on Vosevi. Almost 1/2 through treatment. I think they will ask for a week 8 load test so next weeks bloods will be Interesting and important, and did I mention I will be asking for 8 more weeks of treatment, extending it out to 24 weeks. Im too far into this thing to just stop at 16 weeks then cross my fingers and hope it goes away! The POLARIS 5 TRIAL must be successful. I want to see 100% SVR ( 99% aint gonna do it!! )  Back in the day- (Just last year that is) relapses were recommended 24 weeks of re-treatment no matter what they used to treat. Seeing how this is my trial, I get to make the decisions around hear, so 24 it is!!! Ok everyone start placing your bets on weather or not I get my 24!!LoL!!  I havent driven my car in 10 weeks, we did not take it over to Seattle for the L/T. I would have been a complete mess worrying about door dings and city traffic. Over there you need a YUGO to get around in, smaller is better in this case,the parking is terrible, the streets ar soo narro. Even with a handicap permit you half to drive around looking for a spot. If I lived in the big city I wouldent even use a car.Thank god for UBER and LYFT. You could spend 20.00 bucks a day and get anyware you wanted to go in the city. Parking at the hotel is 30 bucks a night, what a rip off. as if the 300.00 nightly room charge isn't enough. Im starting to ramble so its time to check out.   RC

Those are good responses from one injection! I took two per week for six months and my results were just above acceptable. I do believe you touched on why it's taking a while longer to achieve your first undetected VL. All of those anti rejection drugs attacking your immune system are most certainly responsible. Hence the reason for adding four more weeks of the good stuff. You will see the Dragon vanquished, but the response times had to be altered to compensate for everything else you've been going after. You'll get there, we all know it and I know you won't be satisfied until you're dancing on it's grave.

Good morning Canuck.    My one and only Nuprogen shot was Last Tuesday, so the results you comment on are 3 days after the shot. I was expecting several shots to get the numbers up, but it only took one!! The third bottle of VOSEVI arrives Monday via UPS. The pharmacy (BriovaRX)calls me and sets up the shipment about 12 days before I run out.I should be EOT mid December (16 weeks) Plenty of time to eradicate the foreign invaders. Yes im fireing  on all 8 cylinders, just having a little turbo lag at the start!! We will see what week 8 has to offer.  RC

Well bro, looks like good news to me! I had to go through your posted labs with a fine tooth comb, and I am still confused, but things look pretty good to me. confuse

Obvious (easy) things to see, were glucose and BUN a tad up, HGB/HCT a tad down (but all of these were only by tads).

The other important bits, that are harder for me to suss, are what appear to be, within normal ranges WCB and neutrophils!

 

The lab report you posted below has the date of Friday, Oct 13 on it, and (if I have the timeline right), you only started your first sub q of neuprogen 4 days agao - so then, did it go like this??:

Day 1 of neuprogen - Weds Oct 11?  - you reported your neutrophils prior to starting neuprogen had been clocked at only .33? 

Day 2? of neuprogen - ?                   - you mentioned neutrophils had increased to still being too low at 1.26?

Day 3 of neuprogen - Friday, Oct 13? - shows (on your posted lab) neutrophils have already increased to be (technically) within normal limits at 2.46?

(If I have this right), then ... that's a very good and fast rebound, isn't it?

 

As far as I can see, you are back on-line, firing on all cylinders again, no?

Things should speed up again in the VL dept - only 70 to go!

Hi All. Well I have the results of week 5 and week 6 viral loads. I?m still not where I wanted to be by now. However on the bright side of things the Neupogen shot I got last week is working, my WBC is solidly in the middle and the Neutrophils are now in the low(very low) normals.  Week 5 V/L= 87 & Week 6 V/L= 70  (for anyone new to this) The VOSEVI stops the reproduction of the HCV VIRUS (several billion a day) so that your Immune systen can kill the virus. Thats a good scenario if your immune system is up and working. Mine is NOT working very well because of all the anti-rejection medicine I take post liver transplant. Im hoping that with the higher white blood cell count and the Neutrophils count coming up I can fight a little harder and get to UND soon. There for a couple weeks I really dident have a immune system to kill the virus. They have added 4 weeks to my treatment-  so 16 weeks total instead od 12 weeks. I am going to ask for 24 weeks and see what they say. I really expected to be undetected by now, but I dident take into consideration all the anti-everything meds I would be on, and how they would adversely effect my immune system. Im going to ask for a week 8 V/L and go from there.    RC

Hey RC,

So, 'bout 5 days ago, you were reporting the result of your 5 week VL being at "about" 87, and good you also said you will get a 6 week VL done as well, which is great, I don't have the exact timelines what these exact dates are, but ... when IS your 6 week VL to be drawn and when do you think you will get those results back?? You do seem to get these results back so fast!

I know you are having to go to the lab SO much, drat, so much jabbing being required, it is a lot, and tough, but all to the right end. Them, keeping tabs on ALL aspects of your army, and counting the inevitable viral casualities. And now, extra sub q drugs to boost your bloods! Fingers crossed, that all the good things we all want to see go up in your bloods, go up quick!, and that we see an ever-lowering VL for your 6 week VL result - it will happen, it's just a matter of when ... that und IS coming! Nice if would come now, but, it looks like you got a whole other 10 weeks to kill off a measely 80-some varmits. You are doing it!!! and we can all bet that UND IS gonna happen right soon! Oh that und is going to feel so good, after all this time, NO ONE deserves those 3 little letters more than you (and Chris) my dear.

I felt lucky too when Gilead followed me so closely during my trial. NOT counting pre-trial VL's or any of the many after EOT ones, I got 4 VL's during my 8 weeks of Vosevi - week 1, week 2, week 4 and week 8 (EOT).

Your bloods are on a whole dif scale tho, emcompassing your many more miracles happening! Worth every poke to see them blossom. biggrin C.

BIG WOOT!!

That's what I wanted to see this morning, yeah buddy! That has to be the largest Log reduction I've ever witnessed in my young (cough) life. It must be that fiber free, miracle liver saying the same thing, "Die Dragon, Die". If you have another VL at week 6, I already know the results will be a big fat zero. What a special day that will be, Brother! Nothing but success in your future....

Now, back to you, speeding along past the mileposts.

I am sure we all wish for everyone to be safe and well, how we wanted you to be well, and how you and chris wish the same for everyone else - but as far as you finally getting your GT3a absentee-card, and pulling the lucky new liver straw, it's too late, it's already a done deal! - you and "buddy" and Chris have already started your "new" lives!, and nothing that the rest of your friends here can take any credit for, except for our well wishes, prayers, freindship and every positive vibe we could muster up to send your way. It was all you and Chris, all the way, winning this race, and what an astounding job of it you have done. 

I made a promise to you too, a while back, that I would see you cured in my life-time - thank you for helping me to keep that promise and for allowing me to see this happen so soon!   

You remind me of this commercial. You and chris buckled up in those special rocket-type X across the cross the chest seat belts, replete with astronaut type helmets and ear protection, engine screaming, seeing the finish. Part of the beauty of this machine is that it can go from 100 to 0 mph in about 3 seconds flat, which is about the same amount of time you two will be needing to get that UND. biggrin C. (Gilead really should make better commercials!) Just pretend that's you and buddy and chris and vosevi in the red machine and that dumb ball is the last virus so easily outperformed.

496 after only 28 pills is really rather amazing RC.

Of course you'd wish for und but those low LFTs are holding steady and man oh man your body is going through a lot of readjustments and with all your anti rejection meds it's just a matter of time.  I'm glad the Drs are going to do a 6 week test.

Hi all, well I got the news on my Viral Load. at 4 weeks I'm at 469  It's a bummer but they said that was not out of the ordinary for a post transplant patient. I need to remember that I'm taking tons of anti-rejection medicine and thats knocked my immune system way down. Basically all of my virus fighting soldiers (immune system) have been cought off guard by the anti-rejection meds and are too weak to put up a good fight!!   HOWEVER  I can't discount the fact that I have gone from 100,0000000 to only 469 in 4 weeks, after 28 VOSEVI !!!     My sick soldiers won't quit, they will never give up.Kill,Kill,Kill. I'm digging in and prepared to take this as far as I need to.  The doctors are talking about doing a V/L at the 6 week mark. Does anyone remember the lesson we all have learned in the process of fighting HCV?  It's all about being patient.  It's a hurry up and wait game. LFT's are just about the same as the last blood draw, they are AST-15 & ALT-28    With LFT's like this and a low viral count~It's working,It's just going to take a little longer to get to UND   It was disappointing but I'm over it now.   RC

That's a great idea, Mike! I'll hold him down, you know where we have to stick that thruster, right? If RC gets the results before we can get set up, we might not catch him and Chris in that red hot rod of theirs! They'll be flying high and doing 175. We might need the turbo to catch them, ha, ha!!

Brother! Those are fabulous LFT's! You can be completely proud of those and you know, it tells me everything I need to conclude that Vosevi is the magical silver bullet you've been waiting for. Of course that shiny new liver has helped some too, lol! With numbers like that, you should be well aware of the good things going on throughout the Kingdom of Dead Dragons. Treatment is clearly working, congratulations my friend. I fully anticipate the undetected viral load you have been waiting for. We are all excited and look forward to the good news.

One more thing. I'm glad your Sentinel Tree wasn't in my yard two weeks ago. Had it been, you'd be warming up to it in your wood stove this winter! Glad to hear you're back home and things are getting back on track. Nothing better than the comfort of your own recliner. Thanks C.  And yes we did pass the tree. Its a old tree and looks like it has taken a lighting strike or two but its still standing! Rocky slept on the bed all knight and wouldent leave us alone. He slept at the top of the bed on Chris's pillow.He kept a paw on her the hole night! It is nice to be home. Its hard just sitting around,I cant lift anything over 5 pound,cant risk a abdominal hernia. They cut through your six pak to get to your liver. (Like I had a six pak)-I could put up a picture of the incision , i will try to crop a pic that just shows the incision. RC

We made it home today. Rolled in at 7:00 P.M.     Its nice to be home.    My recliner feels soooo Good! Nothing new to report. I will have blood work done locally, on the 26th I do a V/L and should get the results by next Friday . I'm expecting UND!!

I love your re-view back back (AND forward)!, 37 years is a lonnnng time, dealt with SO well and bravely (and you still are), we all know this is not easy. OK-Nothing but good things to report!!    My LFT"s.  AST-15.    ALT-28.  The Doctor said those were the numbers of a 15 year old kid!!!!!!!      My doner was between 14 and 24-Thats all they would telll me about the new liver, however they slipped and mentioned it was from Alaska and male.  I will write the doner family soon. All my labs are great,and they are cutting me loose- We get to go home. I will have lab work done at home and come back to Seattle at the two month's post for clinic, then at the three month post,  V/L next week on the 26th,then who knows?

I'm feeling stronger every day. What an experience this has been. Life changing for sure!
